Quarterly Wage Report/DE9C not required for:

Guideline:

Groups of 6+ enrolled with prior coverage.

Promotion through 12/15/20:

6+ enrolling, but must provide a prior carrier bill. DE9C is also not required for Specialty. 

Virgin groups excluded from promotion.

Guideline:

  • 5 to 95 enrolling
  • Blue Shield reserves the right to require the following documentation for groups of 5 to 95 enrolling:
    • The group’s most recent DE9C Quarterly State Tax Withholding Statement
    • Payroll register for employees hired after the DE9C filing or if any employees are out of state

Guideline:

Groups with 10+ enrolling

Group with a variance of less than 10% between the number of employees enrolling and the number of employees on the most recent prior carrier bill.

Groups with a lapse of coverage of less than 3 months.

Must provide the most recent prior carrier bill for all the above.

DE9C not required for Sole Proprietors.

Promotion through 12/31/20:

HMO only with 6+ enrolling with no participation attestation, prior carrier bill, nor waivers required. No minimum participation requirement. 

HMO package is separate from Enhanced Choice A and Enhanced Choice B plans.

Promotion (no end date):

Groups of 6+ enrolling

Business Documentation required (example: business license/SOI)

Start Up groups and groups leaving a PEO do not qualify for this promotion.

Groups with 5+ enrolling. Must provide prior carrier’s bill, and 80% of employees on prior bill must be listed in present enrollment.

Groups with more than 5 eligible employees with less than 5 enrollees due to valid waivers are still eligible for this promotion.

10+ enrolling: Can submit a completed and signed Participation Certification form in lieu of a DE9C. If in business less than a year, submit at least one applicable tax document.
